looking for a Notice of Withdrawal of Notice of Pendency Action form for California san mateo county

Plaintiff has agreed to sign it to remove cloud on title due to fraud by bank, proved it wasnt me!

All the plaintiff asked is that I prepare the document and his client will sign it! looking for that document!

You should take Mr. Whipple up on his offer. There is no statewide form. A Notice of Withdrawal of Pendency of Action is prepared on pleading paper, using the case information from the actual action that was used to issue the notice of pendency.
