I believe that to become an adult's conservator (or guardian), you will need a court order. That may not be too difficult to obtain, but it is not particularly easy without a lawyer's assistance. Also, be advised that the court, in making you a conservator, will hold you to a high standard of ethical performance and will probably require you to file periodic reports that will be somewhat of a hassle for you. Nevertheless, this may be what you have to do, so more power to you.

Find a local lawyer that has some experience in adult conservatorships and have an initial consultation. If you can't afford a lawyer, in some counties the public defender will handle your legal needs; in others, maybe the county clerk or adult protective services can steer you.

Re: what type of form

Under normal circumstances, an Advanced Health Care Directive would work. However, because your Mom already has Alzhemers, you will likely need to apply to the court for a conservatorship.
