what type of paper work do I fill out in order to put my wife on my two paid in full properties. I want joint tenancy with rights of survivor ship.

A deed is the right "type of paper work" to transfer ownership or part ownership of real estate. I strongly suggest that you go to an escrow company and pay their fee to prepare the deeds properly. Please understand also that when you "put your wife on" these properties, you are making a substantial, valuable gift to her. The two of you should probably engage a tax advisor and a family wealth planning advisor before tearing headlong into moving properties around within the family with no knowledge of the tax, etc. consequences. This may make sense, or it could be a big, big mistake.
