Legal Question in Social Security Law in California
on the proof of service(cv-01) where i had to mail the forms do i fill out only #1,3 & 5 then sign & date, or what?

A litigant is not permitted to sign the proof of service. Have a friend or relative to it. You would fill out the person's address, the name and address of the recipient, check the box that says it is by mail, and have the "server" date and sign it. Then mail it to the recipient in an envelope with affixed postage.
