Legal Question in Traffic Law in California

i missed a court date. my court date was sept 22 2008 i was arrested on august 27 i was in jail until oct 15 . i have proof of incarceration. the court said i also need a form or document to go along with the proof of incarceration. What is the form how do i get it?

Ask the court what form it wants; if the clerk in the criminal division will not help, go to the courtroom in whilch you failed to appear and ask the clerk there what form she or the judge need you to file. If you still can not get an answer, try to see if there are any non-profit groups helping ex-prisoners who might be able to tell you.
