Do I need to do an addendum to my living trust if my daughter, who is a first trustee substitute, has re-married and changed her legal name?

Yes, I would have a short addendum identifying her by her new name notarized and added to the end of your living trust. It can be a short paragraph identifying the paragraph in the trust that allows you to amend/update the trust and then identify the paragraph(s) where she is mentioned and provide her new identifying information.
