affidavit of domicile

My son recently passed away with cancer, I am beneficiary of his sep ira. I am trying to transfer to my name, they are asking for an affidavit of domicile which has me signing that he owes no debts or taxes, which he does have credit card & medical bill debt. He has very little money or other property, but I cannot sign that he is debtfree to transfer the ira to my name, is there another form I can send them?

No idea. I don't think they need or should require anything at all to pay to the named beneficiary other than a death certificate and proof of identification for you.
