The format of the amendment greatly depends on underlying document. It is too great a risk to not draft these correctly, thus you should be skeptical of any attorney who gives you a "one-size-fits-all" form.

You are better off paying a nominal amount to a qualified estate planning attorney who can review the trust and provide you with a properly drafted amendment. Either I or another estate attorney can do this for you.
