I assume that you are the LPS Conservator and need to file a Petition for Reappointment. In Los Angeles, they have a specific form to use. It's probably a different form in your county.

Call the Court or go to the Court's website to see if they have the form for your county. You can also call the County Counsel in your county and ask them. They might be willing to help. You can also ask the Court to appoint an attorney to help you with the Reappointment Petition.

These cases tend to be a lot less adversarial than regular probate cases.
