Changing my daughters last name

I have been with my husband for 5yrs my daughter is 6 and has been raised by my husband. My daughter has never seen or talk to her father on the phone,my husband is the only father she has known all her life. We have two other children and we want to change my daughter's last name from my maiden name-her father's last name (she carries both names)what would be the fastest and best action for me to take?

you can file a petition for name change. it is pretty simple but you will need to give notice to the father.
