Court ordered release of money

What form do I use for a court ordered release of money? My son has a blocked trust fund set up by an insurance settlement and myself. It was deposited into a blocked trust fund several years ago, and now that he is 18, we need to provide the bank with a court ordered release of money form. Can you tell me the form that is needed??

You may need to petition the court for a hearing to release the funds. At the hearing, if the conditions are met (perhaps just turning age 18 in this case), the court should order the funds released to your son.
