Legal Question in Wills and Trusts in California

My dad died last year. We have a court order that says that as the trustee of his living trust that I am in charge and that the trust owns the property. But the records in the county for the property are still in his name. How do we get that changed?

you file an Affidavit of Successor Trustee and Preliminary Change of Ownership with the County.
