My daughter is physically and mentally handicapp she just turned 18 what forms do I need to file to become her conservator?/guardian?

Since she is over 18, you would need to file for conservatorship. Courts are reluctant to impose a conservatorship, so the burden is on you to establish that other, lesser methods, such as a power of attorney, won't work. Are you receiving services through the Regional Center? If so, I would work with them to help establish your conservatorship. You should also consult an attorney because the process is not easy.

I agree with what Mr. Feldman said. You would have to do a conservatorship. I strongly urge you to speak to an attorney familiar with that area of law, and not file papers on your own. The courts tend to have developmentally disabled adults work through regional center, and strive to give them as much autonomy as possible. This may mean less time with you, and may not be what you are looking for.
