what can i do if i want to disinheirt my neice from my grandmother's will, she know suffers from dementia?

You cannot disinherit a beneficiary from another person's estate planning documents. It sounds like your niece needs a conservatorship and someone will need to petition the court to be appointed her conservator of the estate so they can handle her inheritance.
