Durable Power of Attorney for Incapacitated Grandparent (Not Conservatorship)

My grandfather is mentally and physically incapacitated and I need to obtain various medical records and make certain medical (not financial) decisions for him.

How do I get a durable power of attorney without spending thousands on a conservatorship? Or do I not have a choice?

Unfortunately, if your grandfather is not mentally capable, it is too late to sign a power of attorney, and a conservatorship is required. If he is able to understand what he's signing, then he can do so.
