What form do I have to fill out to get my grandchildrens deceased mother's property from her parents?

It depends on how it is held - in trust? In a custodial account? In a guardianship? In a probate estate?

You really need to sit down with a lawyer to go over the facts and any documents in detail. Was there a will? What is the extent of the property? How was title to property held (as noted by the first response)? You may need to open a Probate. Unfortunately, from your limited facts I have more questions than answers.
