Is there a particular form which needs to be filled out or followed for us to make a change (codicil) to our Trust?

Since most trusts or wills are customized documents, there is no form per se that would specifically deal with your situation, although you might be able to find "off the shelf" fill-in-the-blank forms that may work. Normally, a trust amendment or codicil is something that a law office would prepare for clients. It all depends on what you are comfortable with, and willing to spend an extra bit of money for your peace of mind. I normally advise having the changes done by an attorney.
