I want to add a codicil to my will for 5 grandchildren. Do I need a separate codicil for each, or will one form do for all five?

Generally speaking (because I don't know the terms of your will) one codicil should be sufficient. However, please make sure the codicil is properly executed with all the same formalities of a will.

Unless the will states otherwise, one codicil will suffice. Please be careful with store bought legal documents, they often do not have the same legal consequences that people expect when they purchase them.
