Legal Question in Wills and Trusts in California

My husband and I are joint tenants along with my mother and father on a deed of property in Californian. Both of my parents have passed away, my mother in 2007 and my father in November 2012. We did not fill out a change in ownership for my mother and the county did not catch the error as well. I need to know if I have to do a separate affidavit for both my parents or can it be combined on one?

You can do one affidavit with both death certificates attached. Don't forget to file the Claim for Exclusion from Reassessment.
