Medical Power of Attorney

My father was told to get a medical power of attorney, which forms does he need to fill out, that covers everthing.

My advice if financially feasible would be to have estate planning done, including health care issues, but there is a form from the California Medical Assoc. at http://www.cmanet.org/bookstore/category.cfm?catid=12 you could look into if money is tight.

You can find a California form online, and I'd look for a HIPAA release as well (can be separate or included in the document). Because this covers only medical decisions, though, I'd suggest making sure all other estate planning bases are covered at the same time, like a durable power of attorney for property and an updated trust and/or will.
