Legal Question in Wills and Trusts in California

My mom wants to give me her car. The car is already in my name because I had to travel to buy it and she couldn't be there. Is there any form I need to fill out?

No - there is no legal DMV form. You probably would want to get a letter of release from her indicating that she has no interest in the vehicle and no loan or any type of lein is owed just in case she passes away and someone attempts to claim it is hers. Also make sure it is registered in your state and you have it insured in your name - that is about it!
