What power of attorney form do you use that is still validate after death?

None. A power of attorney is only valid during the life of a person. You would need to establish a trust with a successor trustee to handle the estate.

Ken is right. There is no power of attorney valid after a person's passing. A durable power of attorney remains valid when a person is incapacitated, but it does not last beyond death.
