Legal Question in Wills and Trusts in California

I am having a rough time picking just the right form for what is needed in my case.
My mother would like to put a deed back into her family trust that she took out for refinancing a few years ago. I am her daughter and she wants to be sure it will be transferred to the trust so as i will be able to avoid probate. She will not hire an attorney and says a quitclaim deed will sufice. The title is under Carl H. & Judith E.--name removed--(joint tenant) currently. But my father has passed away now. What format should use.? Both of their names or just hers , to Family Trust?

She needs to First do a affidavit of death of joint tenanat, then a quit claim deed from her to the trust.
