I want to be released as attorney in fact from a power of attorney, is there a form?

There probably is not, but all you need do is prepare a similar looking document saying that you decline to so act as to the power of attorney dated XX, you withdraw from any powers or rights, and you have notified the other person. Attach a proof of service showing you sent a copy to the person you previously were actilng for.
