When a request for dismissal has been filed with the court by an attorney (CIV 110), is not a CIV 120 Notice of Entry of Dismissal & Proof of Service required & also copies of such be sent to concerned parties, or notified of such. Thanks

Usually a copy of the request for entry of dismissal is served by mail. Depending on the court's local rules, some clerks do the notice of entry of dismissal, and mail copies to all parties. Some clerks require self addressed stamped envelopes for this, and some also require the form that you mentioned.
