how do I revoke a living trust, and do I need any forms

The trust itself will or SHOULD have section on how to revoke it. Generally, a trust may be revoked by any method provided in the trust document (Prob C �15401(a)(1)) or by a writing to that effect (other than a will), signed by a settlor and delivered to the trustee during the settlor's lifetime (Prob C �15401(a)(2)). If the trust document explicitly establishes an exclusive method for revocation, however, that procedure must be followed. Prob C �15401(a)(2) If you mean instead, how do you amend it to change it, that is different. Keep in mind that any pour over will you did will also have to be changed.
