What can be done if a settlor to a revocable trust becomes incapacitated and trustee for revocable trust has grant deed giving herself property, with incapacitated settlor's signature?

I am assuming that you are not the Trustee and a beneficiary of the trust. If the trust does not state the Trustee is to receive the property, a beneficiary can file a petition to remove the Trustee and rescind any and all documents signed by the incapacitate settlor.
