Re: Signing as Power of Attorney

The simple answer is, "YOUR NAME" attorney in fact for "THEIR NAME." However every one that takes such a signature by you will want to see the Power of Attorney, and to make a copy for themselves. And for checks the bank may insist on a Special Power of Attorney for checks. And the checks may have to be sent (deposited) through the note department. (B of A used to insist on that, and it's been a decade plus since I had to do it.) And if you're going to approve medical care you'll need a Health/Medical Power of Attorney.

Sign the principal's name, then the agent's name, then "under durable power of attorney dated xxxxx."
