Small Claims Affidavit

Ohio Unclaimed Funds,Life Insurance Policy, request a Small Claims Affidavit be sent. Does California have such a form and if so, how do I obtain it. What is involved in completing the form?

I have never heard of such a document. See if a notarized [will cost $10][call up Small Claims Court and ask them if such a form exists]sworn declaration is sufficient. At the beginning you need to put in language to the effect "I, Gene Ford, do hereby declare: at the end you should have "I, Gene Ford, under the penalty of perjury and/or rules of the State of Califronia, do hereby declare athat the above is truth and correct of my own knowledge and belief, and if called to testify I would and could so state. Signed on the 21th of June, 2008, at Oakland, California.

An affidavit and declaration are the same.

I have no idea what that is. If the funds are on an insurance policy, one would need to prove the identity of the beneficiary to make the claim. Are you sure you got that right?
