title on trust

If John and Jane Smith have a living trust, titled The Smith Living Trust,

John Smith and Jane Smith, trustees of the Smith Living Trust, dated June 10, 1995, they have their bank acct. title John Smith and Jane Smith, Trustees of the Smith Living Trust, dated June 10, 1995. Then John Smith passes and the bank wants to update the bank account. How should the bank title be written. Is it Jane Smith, trustee of the John Smith and Jane Smith living trust (which keps the original title), or is it just Jane Smith, trustee of the Smith Living Trust, dated June 10, 1995. The question is, how does the bank accounts or other titles, including property now supposed to read once Mr Smith passes. For instance, should it also be changed on the property title.

Is Jane Smith now the trustor only.
