What is a verified supplement. Is this a form or is it done on pleading paper showing the correction.

I am not familiar with that term. I think you mean an "amended" motion or whatever the name of the document is. All matters, except letters, sent to the Court are on pleading paper. Verified means it is under penalty of perjury; if you are submitting what you claim are facts as opposed to legal argument, it must be verified.
