Legal Question in Workers Comp in California

when 4850 time runs out, what happens?

If I injure my back at work and it requires extensive rehab while I am off duty I am on �4850� time and getting a full paycheck for up to 1 year per injury. Now, what if I don�t rehab before that 1 year is up or if I re-injure by back over my career and I use my 1 year that is allowed for that injury, what options does the city have? Ie. Work Comp. Payments at non-safety rates (80%??), no pay, medical retirement, ect???
