Legal Question in Workers Comp in California

i dismissed my attorney and my claim adjuster for my workers compensation case cant talk to me untiil i fill out and send a substituion of attorney form but how do i fill it out if i dont have a new attorney?

On the form, you substitute yourself in as your new attorney. Soyou need your old attorney to sign agreeing he will no longer be attorney of record, you sign agreeing, and you sign as accepting the case as substituted in attorney.
