Who's doctor do I have to use
Do I have to use my employers doctor on a workers comp. claim or can I use my HMO doctor thru my healthcare provided by my work or is this up to my employer?

Good Question
For the first 30 days the employer has the right to designate the doctor, unless you have pre-designated your private physician as your treating doctor.
Also at issue is the date of injury and whether the employer has an MPN. If so, then you probably will be forced into the MPN, but you have the right to change doctors within the group.
For something like this, I would ask my doctor just to get a second opinion.
