Legal Question in Workers Comp in California

My employer is unwilling to provide information

My doctor told me a week ago that pain caused in my wrists shoulders and arms was a work related injury. My employer was notified that day . She has still not provided me with paperwork to fill out and has not provided my doctor with billing information. As far as i know there has not yet been any contact with the employers w.c carrier about this. My employer is trying to make me feel guilty for this , has offered to give me a raise and pay me under the table if i stop the claim, which i am unwilling to do . Who should i consider talking to about this, and what does it mean.

You should request a DWC1 (employees claim form) complete it and return it to your employer for completion of the worker's compensation insurance carrier. You should really consult an attorney, however, as that is just the start of the battle. The worker's compensation insurance companies have attorneys, why don't you? Attorney fees are limited by law to 9% to 12% of the final settlement or award. Call me directly at (619) 222-3504.
