This is to be followed with A CLAIM FORM FOR OCCUPATIONAL INJURY.

Plus treatment in the insurer's Medical Provider Network of docs (NOT your family practitioner or the local Urgent Care guy).

If you aren't getting any response from anybody, I can get you to doc's near West Covina there who can treat you until the Insurer either issues a Denial or an Acceptance letter (the Labor Code says you get $10,000 in treatment until the Insurer either accepts or denies responsibility in writing).

Go Here and read ... this is the state's website at the Comp board, pretty helpful: http://www.dir.ca.gov/dwc/WCFaqIW.html

HERE"S THE FORM that the employer is supposed to furnish and get the process started: http://www.dir.ca.gov/dwc/DWCForm1.pdf

Hope that helped some.
