Legal Question in Workers Comp in California

I was injured on the job in Oct 2009. I received my final treatment in January 2010. My condition has come back and I am no longer employed by the company I filed the claim with. My current employer is telling me that since it is a pre-exiting condition I need to reopen the original claim. Do I need to contact my old HR department? Do I contact the original insurance company?
