Insurance Co Causing Delays On Needed Surgery

I was injured on the job. After the Insurance Co doctors tell me I am lying and faking an injury, an MRI, done by my chosen physician, clearly indicates that I need surgery. My doctor has formally requested authorization seven times. The Insurance Co finally answers back with ''Please explain why.'' After doing so, the Insurance Co lawyers write back with ''we believe this injury was somehow caused back in 1990. Please explain why we should give authorization'' (Their reasons for delaying treatment against the facts are completely ludicrous -and I don't understand how they could ever get away with saying what they say). This has gone on for 3+ months. I am in pain and have been out on TTD for a total of 5 months now. How long can they stall for? Are all these games and nonsense legal? Thanks.
