Does a religious non profit have to pay worker's comp for an employee?

Generally, yes, although there may be exceptions for Directors of the non-profit who are performing work.

All employers must have workers' comp. coverage for their employees [as opposed to owners who are not covered]. The nature of the entity does not matter.
