Am I am able to switch from getting Unemployment Isurance to State Disability now that My Dr has Said that I have a Permenant Disability?

Why were you not on State Disability before? I you were not working because of injuires you should have been collecting state disability.

You could have collected SDI from the beginning of your injury as long as your treating doctor was willing to certify you for disability. Another option would have been demanding temporary disability benefits directly from the insurance company which should have been 2/3 of your salary. You may still switch from unemployment to disability but it may have a negative reflection since unemployment benefits suggests that you are able and willing to work.
