Not given Any WC paperwork - Need to get benefits started

I was injured on 10/23 in a fall from a forklift at work. I have been off work for a week now and have at least another week before I will see the Dr. Again. The cause of my fall was that the carts did not latch properly onto the lift and it slid off taking me with it. This was a known issue and had happened before. AFTER, my accident they ''decommissioned'' the forklift until they could get it fixed. My employer has provided me with no paperwork regarding my WC claim. I am trying to get my temp benefits started but am not having a lot of success. What do I need to do right now, just to get some money coming in to cover my bills?

Go to the employer [the HR Department if they have one], demand to get an employee' first report of work injury, get the WC carrier's number and address and contact them to get the benefits started immediately. If none of that works, contact the Department of Labor, Department of Industrial Relations and get them to contact the employer under threat of sanctions.
