workman comp-state disability

i would like to know when a workman comp is denial.and you file for state disability and you get state disability why is it so hard to get a doctor to complete your continue claim form., so you can still get state disability money. my state disability stop on sept 20, 2008 and i have 20days to get it sign i was disable back on april 7, 2008 by a medical provider doctor. i am still disable. i was suppose to have back surgeon but insurance company denial claim right after my dignose. so how do i get my state disability continue claim sign, so i can receive my state disability check i have herinated disk in my lower back. thank you
