Legal Question in Family Law in Canada

Change of Name

My child is under 6mths old and currently has his fathers surname. We have never been married to eachother and I wish to add my surname to my child also. Does his father have to sign for me to be able to do this? He is refusing, so if he has to what steps should I take to get this done. His dad does pay support and we have never gone to court for a custody agreement. Thanks
