Name change form

I am trying to find a Canadian form which changes my name from the marital name back to maiden name without the benefit of a divorce. I want the maiden name to be legal and on all forms including govt. pension cheques, driver's licence etc.

There is no such thing as "Canadian form"; the law on change of name varies from Province to Province; in British Columbia, you can change your name by either of 2 methods:

(1) After a divorce order in B.C. you apply to the Supreme Court of British Columbia for an Order changing your name; you do not need to publish the request anywhere; the cost is $62 for the motion fee and $31 for a Certificate or Certified True Copy of the Name Change Order, plus legal fees if you hire a lawyer; or

(2) Obtain an application from the Division of Vital Statistics - they will require a criminal records check and publication in the B.C. Gazette, among other things. They can advise you of the cost.

If you do not divorce, you can only change your name by the 2nd method.
