The forms are prescribed by the Attorney-General of BC and are available for either the Provincial Court of BC or the Supreme Cout of BC. For Provincial Court go to http://www.qp.gov.bc.ca/dispute/famrules.htm and scroll down to forms and click there.

For Supreme Court, go to http://www.bclaws.ca/Recon/content/site?id=freeside&xsl;=/Recon/template/toc.xsl/group-C/

and scroll down to - 221/90: Appendix A - Supreme Court Rules under Court Rules Act.

Note: The foms in the Supreme Court are NOT designed to be filled in online or downloaded and filled in; they are prescribed forms you type out yourself, filling in the variable information. Affidavits are prepared by you with the appropriate information typed in by you.
