Legal Question in Personal Injury in Canada

How would you list a non-party witness in a Supreme Court Notice of Application? (I am making an application requesting an order for pre-trial examination of the wife of a guy who hit someone with a golf ball because she was there to witness it and is refusing to answer any questions about the events).
