Is it defamation of character to repeatedly call someone a rapist in front of their wife? Or is that protected by freedom of speech?

No, that is not protected speech, that is per se defamation of character, meaning you needn't even prove damages to collect a judgment against the person making that statement. The only defense they may have against a lawsuit is the truth, meaning they would have to show that you really are a rapist.
