Is there a standard template I should use when I want to dismiss my attorney and retain another?

There is a standard motions heading for each jurisdiction. Any motion that you file must be in that form. It has the name and the address of the court at the top.
There is no required language to fire one attorney and retain another. The language that is most commonly used is "irreconcilable differences."
