If you refuse to testify, the judge can hold you in contempt. In state court, it is generally up to 6 months per violation, but it is probably different in Fed law. The rule is generally that they can hold you until it is obvious that holding you will not get you to testify, but lots of exceptions. Besides, a Clinton witness was held for more than 2 years before being released. If you have a privilege, then the court usually will not find you in contempt.
