does it have to be all correct date age name location and reason of Stop?

It is difficult to prosecute you if the reason for the stop is not correct. Correct date is also important as is correct location. Name can be off as well as age.

Not necessarily. However, certain basic information must be correct.
The courts have held that unless the information is substantially incorrect that would affect your substantial due process rights the ticket would not automattically result in dismissal.
